First Visit
Your initial appointment will consist of any necessary X-rays, diagnosis and a consultation regarding your treatment options. Depending upon the complexity of your care and the doctor’s professional recommendation, treatment may begin at the time of initial consultation.
Please assist us by providing the following information at the time of your consultation:
- Your referral slip and any X-rays if applicable
- A list of medications you are presently taking
- If you have dental insurance, bring your current insurance card. If you don’t have a current insurance card, we will need the subscriber number, group number, and phone number. This will save time and allow us to help you in the processing of your claim.
For your convenience, a medical history form may be downloaded and printed from this website. This form can be completed prior to your first appointment and brought into our office. To download this form, go to the forms section of this website.
IMPORTANT: All patients under the age of 18 years must be accompanied by a parent or guardian at the consultation visit.
Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e. diabetes, high blood pressure, artificial heart valves, artificial joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.).
X-Rays
If your dentist or physician has taken X-rays, please have them forwarded to our office, as they may be helpful in our diagnosis. A digital radiograph can be emailed to us. If additional films are necessary, they can be taken at our facility.
If you are pregnant or suspect you may be pregnant please inform the doctor or staff.
